Job Description

Enhance patient care in the community by performing meaningful home health risk assessments as a Nurse Practitioner. This per diem opportunity allows you to independently visit Medicare Advantage plan members in their homes throughout Clarke County, including Athens and Winterville. Leverage your clinical expertise to make a significant difference in patient outcomeswithout the need to prescribe medications or create treatment plansand enjoy flexibility in when and where you work.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Option to create your own schedule
  • Choose a minimum hourly commitment on a weekly or bi-weekly basis

Key Responsibilities:

  • Conduct comprehensive annual home health risk assessments for Medicare Advantage health plan members
  • Evaluate patient history, current medical conditions, and review all medications
  • Educate patients on their conditions and medication regimens to promote better understanding and compliance
  • Identify care gaps and recommend solutions to optimize patient health and prevent complications
  • Complete each assessment efficiently (average visit time: 45 minutes)
  • No prescribing or treatment plan management involved

Qualifications:

  • Active Nurse Practitioner license in Georgia
  • Current CPR certification (BLS)
  • Board certified through AANP or ANCC
  • Minimum of 6 months of recent NP experience preferred
  • MSN obtained from an accredited nursing program
