Schedule/Hours:

  • Flexible shifts available seven days a weekMonday through Sunday
  • Set and manage your own schedule
  • Commit to a minimum number of hours weekly or bi-weekly

Primary Duties:

  • Complete comprehensive home health risk assessments for Medicare Advantage members
  • Review patients prior and current medical history, conditions, and medications
  • Educate individuals and families on health conditions and proper medication usage
  • Identify care gaps and recommend strategies to optimize health outcomes
  • Focus exclusively on assessment; no treatment planning or prescribing required

Desired Qualifications:

  • Active Nurse Practitioner license in Indiana
  • Board Certified through AANP or ANCC
  • Current CPR certification (BLS)
  • MSN degree from an accredited institution
  • Minimum of 6 months of recent NP experience preferred

Compensation and Perks:

  • Competitive per evaluation compensation: $80$90 for each assessment
  • Mileage stipend and administrative time are compensated
