This per diem opportunity offers flexibility and the chance to serve Medicare Advantage plan members across Bartholomew County, including communities such as Columbus, Hope, and Elizabethtown. Schedule/Hours:
- Flexible shifts available seven days a weekMonday through Sunday
- Set and manage your own schedule
- Commit to a minimum number of hours weekly or bi-weekly
Primary Duties:
- Complete comprehensive home health risk assessments for Medicare Advantage members
- Review patients prior and current medical history, conditions, and medications
- Educate individuals and families on health conditions and proper medication usage
- Identify care gaps and recommend strategies to optimize health outcomes
- Focus exclusively on assessment; no treatment planning or prescribing required
Desired Qualifications:
- Active Nurse Practitioner license in Indiana
- Board Certified through AANP or ANCC
- Current CPR certification (BLS)
- MSN degree from an accredited institution
- Minimum of 6 months of recent NP experience preferred
Compensation and Perks:
- Competitive per evaluation compensation: $80$90 for each assessment
- Mileage stipend and administrative time are compensated
