Seeking a skilled Nurse Practitioner to perform home health risk assessments for Medicare Advantage members in Maury County, TN. This per diem position offers exceptional autonomy and flexibility, making it an excellent opportunity for experienced NPs interested in a dynamic home health environment.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own work schedule
- Minimal weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments for Medicare Advantage health plan members
- Travel to patient residences within Columbia, Spring Hill, Mount Pleasant, and other areas of Maury County
- Assess and document prior/current medical history, ongoing medical conditions, and medication use
- Educate patients on diagnoses and prescribed medications during each visit
- Identify gaps in care and provide recommendations to address them
- Visits may be for annual assessments or post-discharge check-ins
- No requirement to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Board Certification through AANP or ANCC
- Current CPR (BLS) certification
- Master of Science in Nursing (MSN) from an accredited program
- Minimum of 6 months recent experience as an NP (preferred)
- Ability to work independently and comfortably in a home-based care setting
