Key Responsibilities:

• Perform comprehensive home health risk assessments (approximately 45 minutes each), including annual health evaluations and post-discharge visits
• Assess prior and current medical history, medical conditions, and medication usage
• Educate patients on medical conditions, treatment recommendations, and medication usage
• Identify and communicate gaps in care, offering actionable recommendations to enhance health outcomes
• No requirement to create treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in the state of Missouri
• Current CPR certification (BLS)
• Board Certification through either AANP or ANCC
• Master of Science in Nursing (MSN) from an accredited institution
• At least 6 months of recent nurse practitioner experience preferred

This position offers an attractive compensation structure of $80-$90 per evaluation, with an additional mileage stipend and compensation for administrative time.
