Deliver essential care in a flexible role conducting home health risk assessments for Medicare Advantage health plan members across Allen County, IN. Working per diem, you'll engage with patients where they live, gathering important health data and supporting their overall well-being. This position offers autonomy, professional satisfaction, and a tailored schedule to fit your life.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own schedule
- Fulfill a minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual home health risk assessments with each visit averaging 45 minutes
- Assess information including medical history, chronic conditions, and medication usage
- Educate members about their health conditions and prescribed medications
- Identify care gaps and offer actionable recommendations
- Visits may include annual assessments or post-discharge check-ins
- No requirement to create formal treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Indiana
- Current CPR certification (BLS)
- Board Certification via AANP or ANCC
- Minimum of 6 months experience as an NP preferred
- MSN degree from an accredited program
