Job Description

Make a meaningful impact on patient health by providing essential in-home health risk assessments across St. Francis County. This per diem position offers Nurse Practitioners the opportunity to support Medicare Advantage plan members through compassionate, detailed evaluations. Your role will be pivotal in bridging care gaps and guiding patients toward healthier outcomes in their own homes.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling: choose shifts convenient for you
  • Options for minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

  • Conduct in-home annual health risk assessments for Medicare Advantage members
  • Review and document prior/current medical history, conditions, and medication use
  • Educate patients about their conditions and medications, fostering self-management
  • Identify care gaps and make actionable recommendations for follow-up
  • Focus exclusively on assessmentno requirement to prescribe medication or create treatment plans

Qualifications:

  • Active Nurse Practitioner license in Arkansas
  • Board Certification through AANP or ANCC
  • Current BLS (CPR) certification
  • Minimum 6 months of current NP experience preferred
  • MSN degree from an accredited nursing program
