Discover a meaningful opportunity as a Nurse Practitioner conducting impactful home health risk assessments for Medicare Advantage plan members across St. Clair County, Illinois. This per diem role offers unmatched flexibility, enabling you to create your own schedule and work as little or as much as youd like, with shifts available any day of the week. Schedule:
- Work flexible shifts, Monday through Sunday
- Enjoy the autonomy of setting your own schedule
- Meet a minimum weekly or bi-weekly hourly commitment tailored to your needs
Key Responsibilities:
- Complete comprehensive home health risk assessments (approximately 45 minutes each) for Medicare Advantage members
- Evaluate prior and current medical histories, ongoing medical conditions, and medication use
- Educate patients about their health conditions and prescribed medications
- Identify any gaps in care, providing clear and actionable recommendations
- No requirement to prescribe medications or develop treatment plans
Qualifications:
- Active Nurse Practitioner license (IL)
- Current CPR certification (BLS)
- Board Certification via AANP or ANCC
- MSN from an accredited nursing school
- At least 6 months of clinical experience as an NP preferred
This role provides a competitive per-evaluation rate of $80$90, complemented by a mileage stipend and compensation for administrative time, ensuring your dedication is recognized and rewarded.
