Healthcare facilities in the Collinsville, IL area are seeking skilled Nurse Practitioners for per diem home health risk assessments in St. Clair County. This essential role serves Medicare Advantage plan members by performing comprehensive evaluations during home visits, each typically lasting around 45 minutes. Your visits may include annual health assessments or post-discharge checkups, with flexibility in location throughout St. Clair County, covering areas such as Belleville, OFallon, and East St. Louis. Scheduling is flexible, allowing you to create your own work calendar with shifts available every day of the week and a minimum hourly commitment on a weekly or bi-weekly basis.

Key Responsibilities:

  • Complete in-home annual health risk assessments for Medicare Advantage members
  • Review and evaluate medical histories, chronic conditions, and current medications
  • Educate patients on disease management and medication adherence
  • Identify and document clinical gaps in care, proposing actionable recommendations
  • No requirement to formulate ongoing treatment plans or issue prescriptions

Qualifications:

  • Active Nurse Practitioner license in Illinois
  • Current Basic Life Support (BLS/CPR) certification
  • Board certification through the AANP or ANCC
  • MSN degree from an accredited nursing school
  • At least six months of recent Nurse Practitioner experience preferred

This position offers compensation of $80-$90 per evaluation, with additional mileage reimbursement and pay for administrative time.
