Job Description

This per diem opportunity offers you the autonomy to create your own work schedule with flexible shifts available every day of the week. Minimum weekly or bi-weekly hourly commitment is required, ensuring adaptability to your lifestyle.

Schedule:

Role Overview:

  • Conduct comprehensive annual home health risk assessments for Medicare Advantage members
  • Review and evaluate prior/current medical histories, existing medical conditions, and medication usage
  • Educate patients about their medical conditions and medications
  • Identify gaps in patient care and provide actionable recommendations
  • No medication prescribing or treatment plan creation required

Qualifications:

  • Active Nurse Practitioner license in Illinois
  • Current CPR certification (BLS)
  • Board Certification (AANP or ANCC)
  • Six months of recent NP experience preferred
  • MSN degree from an accredited nursing program
