Deliver vital care in a rewarding per diem opportunity focused on home health risk assessments for Medicare Advantage plan members. This role offers Nurse Practitioners the autonomy to create their own schedules, serving communities in and around College Station and throughout Brazos County, including areas such as Bryan, Navasota, and Wixon Valley. As you conduct thorough in-home evaluations, your expertise will directly contribute to improved patient health outcomes and system-wide quality of care.

Schedule:

• Flexible shifts available any day, Monday through Sunday
• Design your own work schedule
• Choose minimum weekly or bi-weekly hourly commitments that fit your lifestyle

Key Responsibilities:

• Perform thorough home health risk assessments for Medicare Advantage members
• Review and evaluate patients current and previous medical history, including medical conditions and medication usage
• Educate patients on their medical conditions and prescribed medications
• Identify care gaps and make informed recommendations for follow-up
• No requirement to create treatment plans or prescribe medications, focusing exclusively on assessment and education

Qualifications:

• Active Nurse Practitioner license in Texas
• Current CPR certification (BLS)
• Board certification from AANP or ANCC
• MSN from an accredited nursing program
• Minimum six months of Nurse Practitioner experience strongly preferred

Per evaluation, compensation ranges from $80 to $90, with additional mileage stipends and compensation for administrative tasks, making this an ideal position for Nurse Practitioners interested in flexible, meaningful work.
