Bring your clinical expertise to a per diem opportunity serving individuals in their homes throughout Whitman County. As a Nurse Practitioner specializing in home health risk assessments, youll make a real impact by evaluating Medicare Advantage plan members in Pullman, Colfax, Palouse, and neighboring areas. This engaging role is structured for flexibilitychoose your shifts and manage your workload with a minimum weekly or bi-weekly hourly commitment.

Schedule:

• Flexible shifts available any day of the week (MondaySunday)
• Create your own schedule to fit your needs
• Assignments may rotate among locations in Whitman County

Key Responsibilities:

• Conduct annual and post-discharge home health risk assessments of Medicare Advantage members
• Review prior and current medical history, chronic conditions, and medication usage
• Offer patient education regarding diagnoses, treatments, and medication regimens
• Spot gaps in care and document recommended solutions
• Evaluation-focused care onlyno development of treatment plans or prescribing required

Qualifications:

• Nurse Practitioner license (WA, active and in good standing)
• Board certification (AANP or ANCC)
• MSN from an accredited nursing program
• Current CPR certification (BLS)
• At least 6 months of recent Nurse Practitioner experience preferred
• Home health experience highly valued
