Deliver exceptional home-based care as a Nurse Practitioner specializing in home health risk assessments. This per diem position offers rewarding flexibility and the opportunity to make a direct impact in Rensselaer County, including Troy, Rensselaer, and Hoosick Falls. You'll be conducting visits with Medicare Advantage plan members in their homes, helping to ensure ongoing wellness and continuity of care.

Schedule:

• Flexible shifts available Monday through Sunday

• Minimum weekly or bi-weekly hourly commitment, based on your availability

Key Responsibilities:

• Perform comprehensive annual and post-discharge home health risk assessments for Medicare Advantage members
• Review and evaluate prior/current medical histories, health conditions, and medication use
• Educate patients on their health conditions, treatment goals, and medication management
• Identify potential gaps in care and recommend actions or follow-up
• Focus strictly on assessmentno requirement to prescribe medications or devise treatment plans

Qualifications:

• Active Nurse Practitioner License in NY
• Current CPR (BLS) certification
• Board certified via AANP or ANCC
• Minimum of 6 months experience as a Nurse Practitioner is preferred
• Masters (MSN) degree from an accredited nursing program
