As a Nurse Practitioner specializing in home health visits, you'll play a crucial role in conducting thorough risk assessments that influence each patients overall wellness and care continuum.

Schedule:

• Flexible shiftschoose what works best for you, Monday through Sunday
• Set your own schedule with minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

• Perform comprehensive annual health risk assessments for Medicare Advantage plan members, including medical history, conditions, and medication reviews
• Facilitate visits for both annual health assessments and post-discharge check-ins, each averaging approximately 45 minutes
• Provide patient education on disease processes and medications
• Identify gaps in care or risk factors and offer actionable recommendations to enhance health outcomes
• No prescribing or development of new treatment plans required

Qualifications:

• Active Nurse Practitioner license in New York State
• Board Certification via AANP or ANCC
• MSN from an accredited nursing school
• Current BLS/CPR certification
• Minimum of six months' experience as a Nurse Practitioner preferred
