Play a vital role in patient care by providing home health assessments for Medicare Advantage plan members across Rensselaer County, NY, including Troy, Rensselaer, and Hoosick Falls. As a Nurse Practitioner specializing in home health, you will conduct comprehensive risk assessmentseach averaging 45 minuteshelping to ensure continuity of care and support for patients following discharge or as part of their scheduled annual evaluations. This per diem opportunity offers significant flexibility, with the ability to tailor your work schedule to your personal availability.

Schedule:

• Flexible shifts available between Monday and Sunday
• Set your own working hours
• Choose a minimum weekly or bi-weekly commitment to fit your needs

Key Responsibilities:

• Conduct home health risk assessments for Medicare Advantage members in various locations across Rensselaer County
• Review and evaluate patients prior and current medical histories, medication lists, and chronic conditions
• Offer patient education on ongoing health needs, medication compliance, and disease management
• Identify care gaps and recommend steps to optimize patient well-being
• This role does not involve developing treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner License in NY
• Current CPR certification (BLS required)
• Board Certification through AANP or ANCC
• MSN degree from an accredited nursing school
• Minimum of 6 months NP experience preferred, but not required
