Deliver comprehensive care in the community by performing home health risk assessments for Medicare Advantage plan members residing throughout Rensselaer County, including Troy, Rensselaer, and Hoosick Falls. As a Nurse Practitioner in this per diem opportunity, youll enjoy a flexible schedule tailored to your availability, providing invaluable health insights and education during visits.
Schedule
- Flexible shifts available between Monday and Sunday
- Create your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities
- Conduct thorough annual or post-discharge home health risk assessments, each averaging 45 minutes per visit
- Review and evaluate members medical histories, current conditions, and medication profiles
- Offer patient education on ongoing health conditions and medication management
- Identify and address gaps in care, recommending appropriate follow-up or resources
- No requirements to create treatment plans or prescribe medications
Qualifications
- Active Nurse Practitioner license in New York
- Board Certification via AANP or ANCC
- Current BLS CPR certification
- MSN degree from an accredited nursing program
- Minimum of 6 months recent Nurse Practitioner experience preferred
You will receive compensation of $80-$90 per completed evaluation, plus a mileage stipend and reimbursement for administrative time.
