Job Description

Advance your Nurse Practitioner career by conducting home health risk assessments in a dynamic per diem role serving Medicare Advantage plan members across Kootenai County. As part of a dedicated healthcare team, you will help facilitate preventive, patient-centered care directly in members homesincluding locations in Coeur dAlene, Post Falls, and Hayden. Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling to fit your availability
  • Minimum weekly or bi-weekly hourly commitment

Core Responsibilities:

  • Perform annual and post-discharge home health risk assessments
  • Review and evaluate medical history, current conditions, and medication profiles
  • Identify care gaps and provide actionable recommendations
  • Deliver patient education on health conditions and medication adherence
  • Document findings efficiently; no treatment planning or prescription duties required

Qualifications:

  • Active Nurse Practitioner license in Idaho
  • Current CPR (BLS) certification
  • Board Certification (AANP or ANCC)
  • MSN degree from an accredited nursing program
  • At least six months recent NP experience preferred
