Enjoy a dynamic, patient-centered role as a Nurse Practitioner delivering essential home health risk assessments in Wise County, VA. This per diem position is ideal for experienced Nurse Practitioners who are passionate about community-based healthcare and value professional autonomy. You'll serve a diverse patient population, focusing on Medicare Advantage plan members residing in areas including Coeburn, Wise, and Big Stone Gap.
Schedule:
- Flexible shifts available Monday through Sunday

- Minimum weekly or bi-weekly hourly commitment to fit your needs
Key Responsibilities:
- Conduct home visits to complete annual health risk assessments for Medicare Advantage plan members
- Evaluate and document patient medical history, current conditions, and medications
- Educate patients regarding their health status, conditions, and prescribed medications
- Identify potential gaps in care and provide recommendations for improved patient outcomes
- No requirement to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Virginia
- Valid CPR certification (BLS)
- Board Certification through AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited institution
- Minimum of six months' current Nurse Practitioner experience preferred
