Job Description

Seeking a skilled Nurse Practitioner with home health experience for per diem assignments in Bonner County, Idaho. This opportunity involves conducting in-home risk assessments for Medicare Advantage plan members in areas such as Sandpoint, Ponderay, and Priest River. Each assessment averages 45 minutes, covering comprehensive health reviews and supporting patient education. Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own schedule to fit your availability
  • Choose a minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform annual home health risk assessments for Medicare Advantage members
  • Assess patients medical history, current conditions, and medication use
  • Educate patients on their diagnoses and prescribed treatments
  • Identify gaps in care and recommend appropriate next steps for resolution
  • Visits may include annual assessments or post-discharge check-ins
  • No requirement to create treatment plans or prescribe medication

Qualifications:

  • Active and unrestricted Nurse Practitioner license in Idaho
  • Board Certification via AANP or ANCC
  • Current CPR certification (BLS)
  • MSN degree from an accredited nursing program
  • Minimum of 6 months' experience as an NP preferred
