Seeking an experienced Nurse Practitioner skilled in Home Health for a per diem role performing home-based risk assessments. This opportunity allows you to create your own schedule and make a direct impact by supporting Medicare Advantage members in their homes.

Schedule:

• Flexible shifts available Monday through Sunday
• Option to design your own work schedule
• Minimum weekly or bi-weekly hourly commitment

Responsibilities:

• Conduct comprehensive home health risk assessments for Medicare Advantage plan participants
• Evaluate and document prior/current medical history, chronic conditions, and detailed medication use
• Deliver patient education regarding medical conditions and safe medication use
• Identify gaps in care or follow-up and provide clear recommendations
• Perform a variety of visit types, such as annual wellness assessments and post-discharge visits
• Assessments will not require prescribing medication or developing new treatment plans

Qualifications:

• Active Nurse Practitioner license in South Carolina
• Board Certification through AANP or ANCC required
• Graduation from an accredited MSN program
• Current CPR certification (BLS)
• At least 6 months of NP experience is preferred
• Proven ability to work independently and provide compassionate, patient-centered care

Compensation for this position offers $80$90 per completed evaluation, with an additional mileage stipend and coverage for administrative time.
