Advance your clinical skills by providing essential home health risk assessments in a per diem capacity. This opportunity allows Nurse Practitioners to make a meaningful difference within Marlboro County while enjoying flexibility and autonomy. Serving Medicare Advantage plan members, you will travel to patient homes around Bennettsville, McColl, and Clio for evaluations that foster proactive and holistic care.
Schedule:
- Flexible shifts offered Monday through Sunday
- Self-scheduling for optimal work-life balance
- Minimum weekly or bi-weekly hour commitments available
Responsibilities:
- Perform comprehensive annual and post-discharge health risk assessments in patients homes
- Review and evaluate prior and current medical and medication histories
- Educate patients on health conditions, medication regimens, and overall care
- Identify care gaps, offering recommendations to enhance patient outcomes
- Focus exclusively on evaluationno diagnosis, treatment planning, or prescribing required
Qualifications:
- Active South Carolina Nurse Practitioner license
- Board Certification (AANP or ANCC)
- Current Basic Life Support (BLS) certification
- MSN from an accredited nursing program
- Preferred minimum of six months NP experience in any clinical setting
