Nurse Practitioners with home health experience are needed for per diem opportunities conducting home health risk assessments throughout Shawano County, WI. This position involves direct patient interaction with Medicare Advantage plan members in their homes, providing valuable clinical assessments and education while offering the flexibility to create your own work schedule.

Schedule:

• Flexible shifts available Monday through Sunday
• Self-scheduling: choose shifts that fit your lifestyle
• Minimum weekly or bi-weekly hour commitment

Key Responsibilities:

• Complete comprehensive home health assessments, averaging 45 minutes per visit, for Medicare Advantage members
• Review and evaluate medical, surgical, and medication histories during home visits
• Educate patients about their health conditions, medications, and preventative measures
• Identify potential gaps in care and provide recommendations to ensure optimal patient outcomes
• No need for treatment plan development or prescribing of medications

Qualifications:

• Current Nurse Practitioner license in Wisconsin
• Board Certification through AANP or ANCC
• Active CPR certification (BLS)
• MSN degree from an accredited nursing program
• At least 6 months of NP experience preferred
• Experience with home health or risk assessment visits is a plus
