Key Responsibilities:
- Perform comprehensive home health risk assessments for plan members
- Review and evaluate prior and current medical history, conditions, and medication use
- Educate patients about their medical conditions and medications, empowering them to manage their health
- Identify care gaps and provide recommendations to support improved outcomes
- Focus solely on assessmentno diagnosis, treatment planning, or prescribing required
Desired Qualifications:
- Current Nurse Practitioner License in Oklahoma
- MSN degree from an accredited nursing school
- Board Certification from AANP or ANCC
- Current CPR certification (BLS)
- At least 6 months of nurse practitioner experience preferred
You will conduct each risk assessment visit in a patients home, with each evaluation typically lasting around 45 minutes. Locations may vary and include communities such as Arapahoe, Clinton, Thomas, Weatherford, and other areas throughout Custer County.
This role offers $80$90 per completed evaluation, in addition to a mileage stipend and pay for administrative time.
