Step into a dynamic per diem role performing home health risk assessments for Medicare Advantage members in Custer County, OK. Locations may include Weatherford, Arapahoe, Clinton, and Thomas, with visits averaging 45 minutes each.

Schedule:

• Flexible shifts available between Monday and Sunday
• Create your own schedule
• Minimal required hourly commitment, weekly or bi-weekly

Key Responsibilities:

• Perform annual and post-discharge home health risk assessments for Medicare Advantage members
• Review prior and current medical history, health conditions, and medication usage
• Educate patients about their medical conditions and medications during each visit
• Identify gaps in care and make recommendations to improve patient outcomes
• Documentation onlyno treatment plans or medication prescribing required

Qualifications:

• Active Nurse Practitioner license in Oklahoma
• Current CPR (BLS) certification
• Board certification through AANP or ANCC
• MSN from an accredited nursing school
• At least 6 months of current Nurse Practitioner experience preferred

This per diem position provides pay of $80-$90 per evaluation, alongside stipend for mileage and reimbursement for administrative time.
