Key Responsibilities:

• Conduct home health risk assessments for Medicare Advantage plan members
• Assess comprehensive medical histories, current medical conditions, and medication regimens
• Provide patient education regarding health conditions and prescribed medications
• Identify and address potential gaps in care; offer recommendations for improved outcomes
• Focus is solely on assessment and educationno requirement for creating treatment plans or prescribing

Qualifications:

• Active Nurse Practitioner License in NY
• Board Certification through AANP or ANCC
• Current CPR certification (BLS)
• MSN degree from an accredited nursing school
• At least 6 months of recent Nurse Practitioner experience preferred

Compensation for this role is structured as $80-$90 per completed evaluation, with an additional stipend provided for mileage and administrative time, ensuring your efforts are rewarded beyond direct patient interaction.
