Enjoy the flexibility of a per diem Nurse Practitioner role focused on home health risk assessments within Oneida County, NY. This position allows you to make a tangible impact as you visit Medicare Advantage plan members in their homesserving communities including Utica, Rome, Whitesboro, and New York Mills. Each visit typically lasts 45 minutes and may be an annual health assessment or a post-discharge check-in, with no ongoing treatment planning or medication prescribing required.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage members
- Review prior and current medical history, including medical conditions and medications
- Provide clear education to patients regarding their health conditions and prescribed medications
- Identify gaps in patient care and make recommendations for improvement
- Complete thorough documentation; no requirement to implement treatment or prescribe medications
Qualifications:
- Active Nurse Practitioner license in NY
- Current CPR (BLS) certification
- Board certification via AANP or ANCC
- Six months or more of Nurse Practitioner experience (preferred)
- MSN from an accredited nursing school
