Job Description

Take your clinical expertise on the road by providing patient-centered care in a per diem capacity as a Nurse Practitioner conducting home health risk assessments. This unique opportunity offers you complete flexibility over your schedule while making a significant impact in the lives of Medicare Advantage plan members throughout Oneida County, including Utica, Rome, Whitesboro, and New York Mills.

Schedule:

  • Flexible shifts, Monday through Sunday
  • Set your own schedule to accommodate personal and professional commitments
  • Minimum weekly or bi-weekly hourly engagement

Key Responsibilities:

  • Perform annual or post-discharge home health risk assessments, averaging 45 minutes per visit
  • Assess and document medical histories, conditions, and medication usage
  • Educate patients and families about chronic and acute health conditions, emphasizing medication adherence and understanding
  • Identify care gaps and share actionable recommendations for improved health management
  • No prescribing or treatment plan creation required

Qualifications:

  • Active Nurse Practitioner license in NY
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • At least 6 months of current nurse practitioner experience preferred
  • MSN degree from an accredited nursing program
