Schedule:

• Flexible shifts available Monday through Sunday
• Design your own schedule
• Minimum required commitment weekly or bi-weekly

Key Responsibilities:

• Complete in-home risk assessments for Medicare Advantage health plan members around Parke County
• Assess current and past medical history, chronic conditions, and medications
• Educate patients regarding their health conditions and proper medication use
• Recognize gaps in care and suggest actionable recommendations for improvement
• No ongoing treatment planning or medication prescribing requiredfocus is on patient evaluation and risk assessment

Qualifications:

• Active Nurse Practitioner license in Indiana
• Current BLS (CPR) certification
• Board certification from AANP or ANCC
• At least six months of recent Nurse Practitioner experience (preferred)
• MSN degree from an accredited nursing program

In this per diem role, average assessments take around 45 minutes and may include annual health evaluations or post-discharge visits. Service areas include Adams Township and other Parke County communities such as Rockville, Montezuma, and Marshall. Scheduling may vary, and you could work in any of these locations during your assigned shift.

Compensation consists of $80-$90 per completed evaluation, with mileage and administrative time included, providing both professional satisfaction and a competitive earnings opportunity.
