Your visits will help ensure holistic, patient-centered care and drive positive health outcomes for local residents.

Schedule:

  • Flexible shifts available seven days a weekdesign your own schedule
  • Minimum weekly or bi-weekly hourly commitment to fit your needs

Role Highlights:

  • Conduct comprehensive annual home health risk assessments within members homes
  • Assess and review individuals past and current medical history, conditions, and medications
  • Provide education regarding diagnoses and medication regimens
  • Identify care gaps and recommend actionable next steps
  • Focus exclusively on assessmentno treatment planning or prescribing required

Desired Qualifications:

  • Active Nurse Practitioner license in Indiana
  • Current BLS (CPR) certification
  • Board Certification (AANP or ANCC)
  • Master of Science in Nursing (MSN) from an accredited program
  • At least 6 months of nurse practitioner experience (preferred)
