Job Description

Leverage your clinical expertise as a Nurse Practitioner in a flexible, per diem role focused on home health risk assessments for Medicare Advantage plan members. This position centers around conducting comprehensive assessments within homes in Vigo County, INincluding Terre Haute, West Terre Haute, and Seelyville. Each visit is designed to provide a thorough evaluation (averaging 45 minutes), focusing on patient medical history, medication review, and education, with the potential to cover annual assessments or post-discharge visits as needed.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule based on availability
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Perform annual home health risk assessments for Medicare Advantage members
  • Evaluate medical history, current health status, and medication use during patient visits
  • Deliver education on medical conditions and prescription regimens to patients and families
  • Identify gaps in care, communicating necessary interventions or recommendations
  • No responsibility for creating treatment plans or issuing prescriptions

Qualifications:

  • Active Nurse Practitioner (NP) license in Indiana
  • Current BLS CPR certification
  • Board certification through AANP or ANCC
  • Minimum of 6 months of recent Nurse Practitioner experience preferred
  • Master of Science in Nursing (MSN) from an accredited school
