In this per diem opportunity, you'll conduct in-home evaluations for Medicare Advantage members in Clay County, IN, covering areas such as Brazil, Clay City, and Knightsville. Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule
- Choose weekly or bi-weekly hourly commitments
Key Responsibilities:
- Perform annual health risk assessments in home settings for Medicare Advantage members
- Evaluate medical history and current conditions, review medication histories accurately
- Educate patients on their medical conditions and treatments
- Identify care gaps and provide appropriate recommendations
- No requirement to create ongoing treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Indiana
- Current BLS (CPR) certification
- Board Certification through AANP or ANCC
- MSN degree from an accredited nursing program
- At least 6 months of experience as a Nurse Practitioner preferred
For each completed evaluation, you will receive between $80 and $90, in addition to a mileage stipend and compensation for administrative time.
