Schedule:

• Flexible shifts available Monday through Sunday
• Create your own schedule
• Choose weekly or bi-weekly hourly commitments

Key Responsibilities:

• Perform annual health risk assessments in home settings for Medicare Advantage members
• Evaluate medical history and current conditions, review medication histories accurately
• Educate patients on their medical conditions and treatments
• Identify care gaps and provide appropriate recommendations
• No requirement to create ongoing treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in Indiana
• Current BLS (CPR) certification
• Board Certification through AANP or ANCC
• MSN degree from an accredited nursing program
• At least 6 months of experience as a Nurse Practitioner preferred

For each completed evaluation, you will receive between $80 and $90, in addition to a mileage stipend and compensation for administrative time.
