Schedule:

• Flexible shifts available Monday through Sunday
• Build your own schedule
• Select a minimum hourly commitment per week or every other week

Key Responsibilities:

• Perform comprehensive home health risk assessments for Medicare Advantage members residing throughout Albany County
• Assess medical, surgical, and medication histories during each 45-minute visit, which may involve annual wellness checks or post-discharge evaluations
• Educate members on existing health conditions and medication regimens to foster understanding and improved outcomes
• Identify and address any gaps in care, providing actionable recommendations for follow-up
• Focus strictly on assessment and educationno treatment planning or prescribing responsibilities

Qualifications:

• Active Nurse Practitioner license in New York State
• Current CPR certification (BLS)
• Board Certified through AANP or ANCC
• MSN from an accredited nursing program
• At least 6 months of recent experience as a Nurse Practitioner (preferred)
