This per diem opportunity specializes in providing at-home risk assessments for Medicare Advantage members in areas such as Clifton Park, Saratoga Springs, and Halfmoon. Schedule Details:

  • Flexible shifts available between Monday and Sunday
  • Autonomy to create your own schedule
  • Minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

  • Conduct comprehensive annual home health risk assessments for Medicare Advantage members in-residence
  • Review and evaluate patient medical histories, current conditions, and medication regimens
  • Provide education and guidance on diagnosed conditions, medication usage, and overall health management
  • Identify care gaps and communicate recommendations to advance patient well-being
  • No requirement for treatment plan creation or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in the state of New York
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • Minimum of 6 months of recent nurse practitioner experience preferred
  • MSN degree from an accredited nursing program

Compensation for this role is competitive, including $80$90 per evaluation, along with a mileage stipend and administrative time support.
