Exciting opportunity for a dedicated Nurse Practitioner to provide high-impact home health risk assessments in Saratoga County, NY. This per diem position allows you to help Medicare Advantage plan members gain deeper insights into their health from the comfort of their own homes. Assessments may include annual wellness visits or post-discharge evaluations and typically require around 45 minutes each. Enjoy the autonomy to build your own schedule with flexible shifts in various locations across Clifton Park, Saratoga Springs, and Halfmoon.

Key Responsibilities:
- Conduct comprehensive annual home health risk assessments for Medicare Advantage members
- Evaluate patients prior and current medical history, ongoing conditions, and medication usage
- Educate members about their health conditions, medications, and preventive measures
- Identify care gaps and provide recommendations to enhance patient outcomes
- Assessments onlyno requirement for establishing treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in New York State
- Current CPR certification (BLS)
- Board certified by AANP or ANCC
- Minimum of 6 months Nurse Practitioner experience preferred
- MSN degree from an accredited nursing institution
This role offers competitive earnings of $80$90 for each completed assessment, plus a mileage stipend and compensation for administrative time.
