Step into a rewarding per diem opportunity as a Nurse Practitioner, delivering home health risk assessments for Medicare Advantage members across Schenectady County, NY. This flexible role empowers you to create your own schedule, with shifts available Monday through Sunday in locations throughout Schenectady, Rotterdam, Glenville, and neighboring communities. Each assessment typically lasts about 45 minutes and varies between annual health evaluations and post-discharge visits.
Schedule:
- Flexible shifts offered any day of the week
- Build your own schedule to meet personal and professional needs
- Minimum hourly commitments can be set weekly or bi-weekly
Key Responsibilities:
- Conduct comprehensive risk assessments in the home setting for Medicare Advantage plan members
- Review patients current and prior medical history, conditions, and medications
- Educate members on their health conditions and prescribed medications
- Identify gaps in care and recommend actionable steps to improve patient outcomes
- Role does not involve developing treatment plans or prescribing medications
Qualifications:
- Nurse Practitioner license in the state of NY
- Active BLS (CPR) certification
- Board certification through AANP or ANCC
- Minimum of 6 months experience as a Nurse Practitioner preferred
- Master of Science in Nursing (MSN) degree from an accredited institution
Nurse Practitioners in this per diem role are compensated at $80$90 per completed evaluation. Additional mileage stipends and administrative time payment reflect your commitment to delivering excellent patient-centered care in the community.
