Seeking a skilled Nurse Practitioner to perform home health risk assessments on a per diem basis within Schenectady County, NY. In this rewarding position, you'll work with Medicare Advantage plan members, conducting assessments in their homesensuring each patient receives personalized and comprehensive care. The average visit takes about 45 minutes and may include annual assessments or post-discharge follow-ups. Locations include Schenectady, Rotterdam, and Glenville, offering a dynamic and varied work experience.

Schedule:

• Flexible shifts available any day from Monday to Sunday
• Opportunity to create your own schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct thorough home health risk assessments for Medicare Advantage members
• Review prior and current medical history, health conditions, and medication usage
• Provide clear education on health conditions and prescribed medications
• Identify care gaps and offer actionable recommendations for improvement
• No responsibility for creating treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in NY
• Current CPR (BLS) certification
• Board certification (AANP or ANCC)
• At least 6 months of recent NP experience preferred
• MSN degree from an accredited nursing school
