Excel in a dynamic position providing home health risk assessments as a Nurse Practitioner across Rensselaer County, NY, including areas such as Troy, Rensselaer, and Hoosick Falls. This per diem role supports Medicare Advantage members by evaluating their health in the comfort of their homes, ensuring high-quality care and guidance.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Choice of weekly or bi-weekly minimum commitment
Key Responsibilities:
- Complete approximately 45-minute in-home risk assessments for Medicare Advantage health plan members
- Evaluate patient histories, medical conditions, and medication usage
- Offer education about medical conditions and medications to patients
- Identify gaps in the patients care and recommend strategies for improvement
- Visits may be for annual assessments or post-discharge follow-ups
- No prescribing medications or treatment plan creation
Qualifications:
- Active Nurse Practitioner license in NY
- Current BLS/CPR certification
- Board Certification through AANP or ANCC
- Minimum of six months experience as a Nurse Practitioner preferred
- MSN degree from an accredited nursing program
