Step into a rewarding healthcare role providing home health services as a Nurse Practitioner, focusing on quality risk assessments for Medicare Advantage members across Rensselaer County. Schedule:
- Flexible shifts available Monday through Sunday

- Commitment minimum determined weekly or bi-weekly
Primary Responsibilities:
- Conduct comprehensive home health risk assessments (approx. 45 minutes per visit) for Medicare Advantage plan members located in Troy, Rensselaer, Hoosick Falls, and nearby communities
- Review and evaluate patients medical, medication, and health history
- Provide clear education to patients regarding their health conditions and prescribed treatments
- Identify care gaps and offer actionable recommendations to improve patient outcomes
- No requirement to develop treatment plans or prescribe medications
Qualifications:
- Valid, unrestricted Nurse Practitioner License in New York State
- Board certification by AANP or ANCC
- Current BLS (CPR) certification
- Minimum of 6 months recent NP experience preferred
- MSN degree from an accredited nursing program
