Job Description

Schedule:

  • Flexible shifts available Monday through Sunday

  • Commitment minimum determined weekly or bi-weekly

Primary Responsibilities:

  • Conduct comprehensive home health risk assessments (approx. 45 minutes per visit) for Medicare Advantage plan members located in Troy, Rensselaer, Hoosick Falls, and nearby communities
  • Review and evaluate patients medical, medication, and health history
  • Provide clear education to patients regarding their health conditions and prescribed treatments
  • Identify care gaps and offer actionable recommendations to improve patient outcomes
  • No requirement to develop treatment plans or prescribe medications

Qualifications:

  • Valid, unrestricted Nurse Practitioner License in New York State
  • Board certification by AANP or ANCC
  • Current BLS (CPR) certification
  • Minimum of 6 months recent NP experience preferred
  • MSN degree from an accredited nursing program
