Job Description

Bring your expertise as a Nurse Practitioner to a meaningful per diem opportunity conducting in-home health risk assessments for Medicare Advantage plan members across Albany County. Working in varied communities, your visits will range from annual health evaluations to post-discharge checks, typically lasting about 45 minutes. This flexible, patient-centered role allows you to create your own schedule with shifts available any day of the week, and only requires a minimum weekly or bi-weekly hourly commitment.

Your responsibilities will include:

  • Performing comprehensive home health risk assessments for plan members
  • Reviewing and evaluating prior and current medical history, ongoing conditions, and medication usage
  • Delivering clear education to patients about their health and medications
  • Identifying care gaps and recommending solutions to support patient well-being
  • Note: This role does not involve creating treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in New York
  • Board Certification through AANP or ANCC
  • Current BLS/CPR certification
  • Minimum of 6 months of recent Nurse Practitioner experience preferred
  • MSN degree from an accredited nursing program
