Advance patient care by performing home health risk assessments for Medicare Advantage plan members across Hudson County, NJ, including Jersey City, Bayonne, and Union City. In this per diem role, you will have the autonomy to create your own schedule and tailor your hours to suit your availability. Each risk assessment averages 45 minutes and could be an annual wellness evaluation or a post-discharge visit, depending on the patients needs.

Schedule:

• Flexible shifts available Monday through Sunday
• Option to design your own schedule
• Commitment to a minimum number of hours per week or every two weeks

Responsibilities:

• Conduct comprehensive health risk assessments in patients homes for a Medicare Advantage health plan
• Review and assess patients current and past medical history, ongoing medical conditions, and medication use
• Educate patients about their health conditions and prescribed medications
• Detect gaps in care and recommend strategies to optimize health outcomes
• No requirement for treatment planning or medication prescribing

Qualifications:

• Active Nurse Practitioner license in the state of New Jersey
• MSN degree from an accredited nursing program
• Board Certification through AANP or ANCC
• Current Basic Life Support (BLS/CPR) certification
• At least 6 months of Nurse Practitioner experience (preferred, not mandatory)
• Reliable transportation for travel within Hudson County

This opportunity offers earnings of $80$90 per evaluation, in addition to a mileage stipend and compensation for administrative time.
