Job Description
Deliver quality care while enjoying flexibility and autonomy as a Nurse Practitioner specializing in home health risk assessments. This position involves conducting comprehensive risk assessments with Medicare Advantage plan members residing in Hudson County, including Jersey City, Bayonne, and Union City. Visits typically take about 45 minutes, with each shift potentially scheduled in any of these locations throughout Hudson County.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule based on your availability
- Choose minimum weekly or bi-weekly hourly commitments to suit your needs
Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage plan participants
- Review patient medical history, past and current medical conditions, and medication lists
- Educate patients about their conditions and medications during the home visit
- Identify care gaps and recommend next steps
- No creation of treatment plans or prescribing medications required
Qualifications:
- Active Nurse Practitioner license in New Jersey
- Current CPR (BLS) certification
- Board Certification through AANP or ANCC
- At least 6 months of recent Nurse Practitioner experience preferred
- Master of Science in Nursing (MSN) from an accredited program
