Embrace a dynamic per diem opportunity as a Nurse Practitioner conducting home health risk assessments throughout Mesa County. This position focuses on providing compassionate, patient-centered care to Medicare Advantage plan members in their homes, including the vibrant areas of Grand Junction, Fruita, and Palisade. Each assessment averages 45 minutes and may include annual health reviews or post-discharge evaluations.

Responsibilities:
- Conduct comprehensive annual home health risk assessments for Medicare Advantage members
- Review and evaluate the patients prior and current medical history, conditions, and medication use
- Deliver patient education regarding health conditions and prescribed medications
- Identify gaps in care and provide actionable recommendations
- No involvement in developing treatment plans or prescribing medications
Qualifications:
- Current Nurse Practitioner license in Colorado
- Valid CPR certification (BLS)
- Board Certification through AANP or ANCC
- Minimum of 6 months experience as a Nurse Practitioner preferred
- MSN degree from an accredited nursing school
