Deliver impactful patient care while enjoying flexibility as a Nurse Practitioner conducting home health risk assessments in Hendry County, Florida. This per diem opportunity allows you to play a vital role in supporting Medicare Advantage plan members in communities such as Clewiston, LaBelle, and Montura.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling to align with your personal and professional needs
- Commitment to a minimum number of hours weekly or bi-weekly
Key Responsibilities:
- Complete comprehensive home health risk assessments for Medicare Advantage plan members, with each visit typically lasting 45 minutes
- Travel to members homes within Hendry County to conduct evaluations that may include annual health check-ups or post-discharge visits
- Review and assess patients prior and current medical history, conditions, and medications
- Provide patient education regarding health status, medical conditions, and prescribed medications
- Identify care gaps, offering recommendations to promote continuity of care
- No requirement to create treatment plans or issue prescriptions
Qualifications:
- Active Nurse Practitioner license in Florida
- Current BLS (CPR) certification
- Board Certification with AANP or ANCC required
- At least 6 months of recent Nurse Practitioner experience preferred
- MSN from an accredited nursing program
This role offers compensation of $80 to $90 per completed evaluation, along with a mileage stipend and additional consideration for administrative time.
