Job Description

Make a meaningful impact on patient care by delivering in-home health risk assessments throughout Glades County. Your expertise as a Nurse Practitioner will help improve health outcomes for Medicare Advantage plan members residing in areas such as Palmdale, Lakeport, and Buckhead Ridge by ensuring timely and thorough clinical evaluations and providing valuable education.

Schedule:

• Flexible shifts available Monday through Sunday
• Create your own schedule
• Minimal weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct annual health risk assessments and post-discharge visits in patients homes
• Evaluate prior and current medical history, review medical conditions, and assess medication history
• Educate patients on chronic conditions, prescribed medications, and healthy practices
• Identify care gaps and make actionable recommendations to improve patient outcomes
• No requirement to prescribe medication or create treatment plans, keeping the focus on assessment and education

Qualifications:

• Active Nurse Practitioner license in Florida
• Board Certification through AANP or ANCC
• Current BLS/CPR certification
• Completion of an MSN degree from an accredited school of nursing
• At least 6 months of recent Nurse Practitioner experience preferred

This per diem opportunity provides excellent flexibility and work-life balance, allowing you to choose shifts that fit your personal schedule. Compensation for this role ranges from $80-$90 per completed evaluation, with a mileage stipend and compensation for administrative time.
