Advance your career and make a meaningful impact in the community by conducting in-home health risk assessments for Medicare Advantage members. This per diem opportunity offers flexibility and autonomy, allowing you to create your own schedule and support patient health across Lake County, Florida.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule
- Minimum weekly or bi-weekly hourly commitment
Responsibilities:
- Perform comprehensive annual risk assessments in patients homes
- Evaluate prior and current medical history, conditions, and medications
- Offer education to patients regarding medical conditions and prescribed medications
- Identify care gaps and deliver recommendations to bridge those gaps
- Engage with members for annual assessments or post-discharge visits, adapting to each patients unique needs
- No prescribing medications or development of treatment plans required
Qualifications:
- Active Nurse Practitioner license in the state of Florida
- Current BLS/CPR certification
- Board certification (AANP or ANCC)
- Minimum of six months recent NP experience preferred
- Master of Science in Nursing (MSN) degree from an accredited nursing school
