Job Description

Deliver valuable care directly to patients homes as a Nurse Practitioner with a focus on home health risk assessments. This per diem opportunity offers autonomy, flexibility, and the chance to make a meaningful impact within Camden County, New Jersey. You will work with Medicare Advantage plan members in the region, conducting thorough health evaluations and supporting continuous improvement in patient care.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Ability to self-schedule and create a work plan that fits your life
  • Minimum weekly or bi-weekly hourly commitment required

Primary Responsibilities:

  • Perform comprehensive home health risk assessments, approximately 45 minutes in length, for Medicare Advantage members
  • Complete evaluations that may include annual health assessments and post-discharge visits
  • Assess medical history, existing conditions, and medication regimens
  • Deliver education on current health conditions and medication management
  • Identify care gaps and suggest actionable recommendations
  • No prescribing or treatment planning required

Qualifications:

  • Active Nurse Practitioner License in New Jersey
  • Current BLS (CPR) certification
  • Board Certification (AANP or ANCC)
  • Minimum of 6 months experience as a Nurse Practitioner preferred
  • MSN from an accredited school of nursing
