Deliver essential care and support to patients in their homes as a Nurse Practitioner providing home health risk assessments on a per diem basis. This position serves Medicare Advantage plan members across Pinellas County, with flexibility to work in locations such as Clearwater, St. Petersburg, and Largo. The role offers maximum autonomy with a truly customizable schedule, allowing you to shape your work around your lifes demands.

Schedule:

• Flexible shifts available any day, Monday through Sunday
• Build your own schedule to fit your needs
• Minimum hourly commitment required each week or bi-weekly

Key Responsibilities:

• Conduct annual and post-discharge home health risk assessments for Medicare Advantage plan members
• Review and assess each patients medical history, chronic conditions, and medication regimen
• Educate patients on diagnosed conditions and prescribed medications, empowering them with knowledge for self-care
• Identify any gaps in patient care and provide sound recommendations for improvement
• No responsibilities for treatment planning or issuing prescriptions

Qualifications:

• Active Nurse Practitioner license in Florida
• Board certification via AANP or ANCC
• Current CPR certification (BLS)
• MSN degree from an accredited nursing program
• At least 6 months of recent experience as a Nurse Practitioner preferred
