Experienced Nurse Practitioners are needed to perform home health risk assessments on Medicare Advantage plan members residing throughout Jefferson County. This per diem position offers flexibility, autonomy, and a meaningful way to impact patient care in the local community. Work is scheduled in various areas within the county, including Le Ray, Watertown, Adams, and Fort Drum.

Schedule:

• Flexible shifts available Monday through Sunday
• Ability to create your own schedule
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Conduct annual home health risk assessments in client homes for Medicare Advantage members
• Assess and review prior and current medical history, conditions, and medication use
• Provide education to members on health conditions and prescribed medications
• Identify gaps in care and make appropriate recommendations for follow-up or resolution
• No responsibility for developing treatment plans or prescribing medications

Qualifications:

• Active and unrestricted Nurse Practitioner license in New York State
• Current BLS CPR certification
• Board Certified through AANP or ANCC
• Minimum of 6 months of Nurse Practitioner experience preferred
• MSN degree from an accredited nursing program
