Deliver impactful care directly to patients homes as a Nurse Practitioner specializing in home health risk assessments. This per diem opportunity empowers you to provide personalized health evaluations to Medicare Advantage plan members across Jefferson County, NY, including Le Ray and nearby areas such as Watertown, Adams, and Fort Drum. Enjoy the flexibility to design your own schedule with a minimum weekly or bi-weekly hourly commitment and shifts available any day between Monday and Sunday.

Key Responsibilities:
- Conduct comprehensive annual home health risk assessments for Medicare Advantage members
- Evaluate prior and current medical history, ongoing conditions, and medication usage
- Educate patients on their medical conditions and medications to enhance understanding and compliance
- Identify gaps in care and provide recommendations to close these gaps
- Note: No treatment plans need creation and no prescription authority is required for this role
Qualifications:
- Active Nurse Practitioner license in NY
- Current CPR (BLS) certification
- Board Certified through AANP or ANCC
- At least 6 months of experience as a Nurse Practitioner is preferred
- Master of Science in Nursing from an accredited institution
This position offers a pay of $80-$90 for each evaluation completed, in addition to a mileage stipend and compensation for administrative time.
