Job Description

Deliver quality patient care while enjoying autonomy and a flexible schedule as a Nurse Practitioner focusing on home health risk assessments throughout Jefferson County, NY. In this per diem opportunity, you'll assess Medicare Advantage plan members in their homes, helping to ensure comprehensive and proactive care. The average assessment takes approximately 45 minutes and may be an annual health evaluation or a post-discharge visit. Locations covered include Le Ray, Watertown, Adams, Fort Drum, and surrounding areas, with schedules tailored to your availability.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Set your own schedule
  • Choose weekly or bi-weekly hourly commitments

Key Responsibilities:

  • Conduct annual and post-discharge home health risk assessments for Medicare Advantage members
  • Review prior and current medical conditions, health history, and medication lists
  • Educate patients on their health status and medication regimens
  • Pinpoint gaps in care and offer recommendations for improvement
  • No treatment planning or prescription duties required

Qualifications:

  • Active Nurse Practitioner license in New York
  • Current CPR certification (BLS)
  • Board certification through AANP or ANCC
  • Minimum of 6 months current Nurse Practitioner experience preferred
  • Master of Science in Nursing (MSN) from an accredited institution

Compensation: This per diem role offers $80$90 per evaluation conducted, with an additional mileage stipend and compensation for administrative time.
