Job Description

Shape the future of patient care by delivering comprehensive home health risk assessments in a dynamic per diem role. As a Nurse Practitioner, you will engage with Medicare Advantage plan members across Wake County, providing essential health evaluations in the comfort of patients homes.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling: tailor your hours week by week
  • Minimum hourly commitment required weekly or bi-weekly

Primary Duties:

  • Complete annual and post-discharge home health risk assessments to support continuity of care
  • Review and document detailed medical, surgical, and medication histories
  • Educate patients about their medical conditions and prescribed medications
  • Spot gaps in care and recommend actions to improve overall health outcomes
  • No requirement to develop treatment plans or prescribe medications

Desired Experience & Credentials:

  • Active Nurse Practitioner license in North Carolina
  • Current Basic Life Support (BLS/CPR) certification
  • Board certification through AANP or ANCC
  • 6 months or more of recent experience as a Nurse Practitioner is preferred
  • Master of Science in Nursing (MSN) from an accredited institution
