Job Description

Step into a key clinical role as a Nurse Practitioner focused on home health risk assessments for Medicare Advantage members. This per diem opportunity offers you the flexibility to design your own schedule and support patient care throughout Wake County, including Raleigh, Cary, and Apex. Each visit averages approximately 45 minutes and may involve annual health assessments or post-discharge evaluations, providing critical insights into members health.

Schedule:

  • Flexible shifts are available Monday through Sunday
  • Create your own schedule to fit your availability
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Conduct annual and post-discharge home health risk assessments for Medicare Advantage health plan members
  • Evaluate prior and current medical history, assess chronic conditions, and review medication usage
  • Educate patients on their health conditions and prescribed medications
  • Identify any gaps in care and recommend appropriate follow-up or resources
  • No responsibility for creating treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner licensure in North Carolina
  • Board certification through AANP or ANCC
  • Possession of a current CPR certification (BLS)
  • MSN from an accredited nursing program
  • At least six months of recent NP experience is preferred

This position offers competitive compensation of $80-$90 per evaluation, with an additional stipend for mileage and administrative time.
