Job Description

Schedule:

  • Flexible shifts available Monday through Sunday
  • Build your own schedule to fit your availability
  • Minimum weekly or bi-weekly hourly commitment

Primary Responsibilities:

  • Conduct home health risk assessments for Medicare Advantage members within various communities across Johnston County, such as Clayton, Smithfield, and Selma
  • Complete thorough annual assessments or post-discharge visits, typically in sessions of approximately 45 minutes
  • Evaluate patients current and previous medical history, review medical conditions and medication use
  • Offer education to patients regarding their conditions and prescribed medications
  • Identify care gaps and provide actionable recommendations to enhance outcomes
  • No development of treatment plans or prescribing responsibilities required

Qualifications:

  • Current Nurse Practitioner license in North Carolina
  • Active CPR certification (BLS)
  • Board certification through AANP or ANCC
  • Master of Science in Nursing (MSN) degree from an accredited institution
  • At least 6 months of experience as a Nurse Practitioner preferred
