Delivering high-quality, patient-focused healthcare in the home setting is vital for ensuring members receive comprehensive care. This per diem opportunity is designed for Nurse Practitioners eager to impact community health while enjoying the flexibility of self-managed scheduling.
Schedule:
- Flexible shifts available Monday through Sunday
- Build your own schedule to fit your availability
- Minimum weekly or bi-weekly hourly commitment
Primary Responsibilities:
- Conduct home health risk assessments for Medicare Advantage members within various communities across Johnston County, such as Clayton, Smithfield, and Selma
- Complete thorough annual assessments or post-discharge visits, typically in sessions of approximately 45 minutes
- Evaluate patients current and previous medical history, review medical conditions and medication use
- Offer education to patients regarding their conditions and prescribed medications
- Identify care gaps and provide actionable recommendations to enhance outcomes
- No development of treatment plans or prescribing responsibilities required
Qualifications:
- Current Nurse Practitioner license in North Carolina
- Active CPR certification (BLS)
- Board certification through AANP or ANCC
- Master of Science in Nursing (MSN) degree from an accredited institution
- At least 6 months of experience as a Nurse Practitioner preferred
