If youre an experienced Nurse Practitioner with a passion for patient-centered care, consider this per diem opportunity conducting home health risk assessments for Medicare Advantage plan members across Johnston County. This rewarding role offers you flexibility and autonomy as you assist individuals in locations such as Clayton, Smithfield, Selma, and surrounding areas.
Schedule:
- Flexible shifts are available any day, Monday through Sunday
- Create your own schedule to fit your lifestyle
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct in-home annual risk assessments; average visit is approximately 45 minutes
- Evaluate members medical histories, current conditions, and medication lists
- Educate clients about their chronic conditions and medications
- Identify care gaps and recommend next steps for optimal health outcomes
- No treatment planning or medication prescribing necessary
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board certification through AANP or ANCC
- Current BLS/CPR certification
- Minimum of 6 months NP experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
Youll earn between $80$90 for each completed evaluation and receive a mileage stipend along with compensation for administrative time.
