Job Description
Make a meaningful impact in patient care by utilizing your advanced clinical expertise as a Nurse Practitioner providing home health risk assessments across Columbus County, NC. This per diem opportunity allows you to support Medicare Advantage plan members by evaluating their health in the comfort of their own homes, contributing valuable information to their ongoing care.

Role Responsibilities
- Conduct comprehensive annual home health risk assessments for Medicare Advantage members
- Evaluate patients prior and current medical history, medical conditions, and medication usage
- Deliver patient education regarding health conditions and prescribed medications
- Identify gaps in care and provide actionable recommendations to ensure optimal patient outcomes
- Visits will average approximately 45 minutes and may include annual assessments or post-discharge follow-ups
- No prescribing or creation of treatment plans required
Required Qualifications
- Active Nurse Practitioner License in North Carolina
- Board Certification through AANP or ANCC
- Current CPR certification (BLS)
- MSN degree from an accredited nursing program
- At least 6 months of Nurse Practitioner experience is preferred
