Take advantage of a unique opportunity to provide important healthcare services in a flexible per diem Nurse Practitioner role focused on home health risk assessments. Leverage your clinical expertise to support Medicare Advantage members by performing individualized assessments in their homes throughout Montgomery County, including Woodlawn, Cunningham, Sango, and surrounding areas.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule tailored to your availability
- Minimum weekly or bi-weekly hourly commitment based on your preference
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage plan members
- Assess prior and current medical history, as well as ongoing conditions and medication usage
- Educate patients on their diagnoses and prescribed medications
- Detect gaps in patient care and provide actionable recommendations for improvement
- Visits may be for annual assessments or follow-up post-discharge; each visit averages around 45 minutes
- No requirement to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Current BLS/CPR certification
- Board Certification via AANP or ANCC required
- MSN degree from an accredited nursing program
- At least six months of current NP experience preferred
