Step into a rewarding opportunity as a Nurse Practitioner conducting home health risk assessments for Medicare Advantage plan members. This per diem position offers the flexibility to create your own schedule, with shifts available throughout the week to suit your availability.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling for optimal work-life balance
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform comprehensive annual health risk assessments in patient homes across the area, including Crossville, Pleasant Hill, and Crab Orchard
- Evaluate patients current and prior medical history, medical conditions, and medication regimens
- Educate patients regarding their medical conditions, medications, and health management
- Identify gaps in care and provide actionable recommendations for follow-up
- Maintain focus on assessmentsno treatment plans or medication prescribing involved
Qualifications:
- Current and unencumbered Nurse Practitioner license in Tennessee
- Active Board Certification through AANP or ANCC
- CPR certification (BLS) required
- MSN from an accredited nursing program
- At least six months of recent Nurse Practitioner experience preferred
You will be compensated $80 to $90 per evaluation, with an additional mileage stipend and payment for administrative time.
