This per diem opportunity offers flexible scheduling in Fentress County, TN, allowing you to create your own work hours and balance your professional and personal life. You will visit Medicare Advantage plan members in their homes across locations like Jamestown, Allardt, and Grimsley, providing vital assessments that support continuity of care.

Schedule:

  • Flexible shifts offered between Monday and Sunday
  • Design your own schedule for optimal work-life balance
  • Satisfy a weekly or bi-weekly hourly commitment as fits your availability

Key Responsibilities:

  • Perform comprehensive annual home health risk assessments for Medicare Advantage plan members
  • Review and document patient medical history, present conditions, and medication use
  • Deliver clear education to patients regarding medical conditions and prescribed medications
  • Detect gaps in care and offer actionable recommendations to address them
  • No medication prescribing or creation of treatment plans requiredfocus is solely on assessment and education

Qualifications:

  • Active Nurse Practitioner License in Tennessee
  • Board Certified through AANP or ANCC
  • Current CPR (BLS) certification
  • MSN degree from an accredited nursing program
  • At least 6 months of relevant Nurse Practitioner experience preferred
