Job Description
Deliver critical care to seniors in the community while enjoying unparalleled schedule flexibility. This per diem opportunity is ideal for a Nurse Practitioner with a passion for home health, engaging directly with Medicare Advantage plan members across Cumberland County, TNincluding the areas of Crossville, Pleasant Hill, and Crab Orchard.
Schedule/Hours:
- Flexible shifts available Monday through Sunday
- Self-schedulingset your own availability
- Minimum hourly commitment on a weekly or bi-weekly basis
Responsibilities:
- Perform home health risk assessments, with each visit lasting approximately 45 minutes
- Review and evaluate patients' prior/current medical history, conditions, and medications
- Deliver education to members regarding health conditions and prescribed medications
- Identify gaps in care and offer actionable recommendations for improved outcomes
- Conduct a variety of visits, such as annual health assessments or post-discharge check-ins
- Note: This role does not involve creating treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in the state of TN
- Board certified by AANP or ANCC
- Current Basic Life Support (BLS/CPR) certification
- Completion of a Master of Science in Nursing (MSN) from an accredited institution
- Minimum of 6 months recent Nurse Practitioner experience preferred
