Job Description

Deliver critical care to seniors in the community while enjoying unparalleled schedule flexibility. This per diem opportunity is ideal for a Nurse Practitioner with a passion for home health, engaging directly with Medicare Advantage plan members across Cumberland County, TNincluding the areas of Crossville, Pleasant Hill, and Crab Orchard.

Schedule/Hours:

  • Flexible shifts available Monday through Sunday
  • Self-schedulingset your own availability
  • Minimum hourly commitment on a weekly or bi-weekly basis

Responsibilities:

  • Perform home health risk assessments, with each visit lasting approximately 45 minutes
  • Review and evaluate patients' prior/current medical history, conditions, and medications
  • Deliver education to members regarding health conditions and prescribed medications
  • Identify gaps in care and offer actionable recommendations for improved outcomes
  • Conduct a variety of visits, such as annual health assessments or post-discharge check-ins
  • Note: This role does not involve creating treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in the state of TN
  • Board certified by AANP or ANCC
  • Current Basic Life Support (BLS/CPR) certification
  • Completion of a Master of Science in Nursing (MSN) from an accredited institution
  • Minimum of 6 months recent Nurse Practitioner experience preferred
